The supplemental chapters were written in interleaf. The cyberleaf conversion of interleaf files to html and gif is not without annoyances. Missed in the conversion are special symbols such as Greek letters, arrows, etc. Also, the current version of html does not have the concept of TAB so some tables appear compressed. There are no "pages" or page numbers in html so footnotes just pop up where they are referenced.
